My wife is 34yrs and is pregnant (approx. 35 weeks). During Ultrasound it is found that the cord is around the neck of the foetus, and the liquor amount is also very less (AFI = 6.7 cm.). Her hemoglobin is also less (9.0). She is carrier of Hepatitis B also. She has to go to her native place for the delivery which is around 600 kms. She is planning to go by train. Please tell us about all the complications involved including of having cord around the neck of the foetus. Is there any risk involved in delivery? May she require pre-mature delivery if liquor status does not improve? Can we take the risk of travel? What are the precautionery and remedial actions to be taken to come out of the situation.

Considering her age, amount of liquor and low hemoglobin it is very important that she delivers in a hospital with good facilities. If the native place has less facilities do not send her there. If inspite of telling, if she insists on going ,she should go as early as possible .It is already late and the fluid may decrease suddenly .she should keep track of the fetal movements and report to the dr. immediately if there is less movement .the effect of cord round the neck can be known only in labour . she may need a cesearean
